Malone's survey of the five kinds of supermind finds that only hierarchies can stop a dangerous superintelligence: ecosystems cannot cooperate, communities and democracies cannot enforce, and paying builders to stop invites extortion. His fix is a democracy overseeing the hierarchy, which he concedes fails because a builder can move to any country without the rule.

Small changes in individuals produced an enormous change in the species, and the effect was invisible at the level where it happened. Part I: the true effect of these cognitive changes was not visible individually, and slightly more creative and flexible individuals aggregated to produce an enormous shift in our capabilities as a group. The measure of that shift is the extinction record. Within a few thousand years of reaching Australia, twenty-three of the twenty-four Australian animal species over 100 pounds were gone; within two thousand years of reaching the Americas, North America lost thirty-four of forty-seven genera of large mammals and South America fifty of sixty. Part I makes the right point about it: ecological devastation is common in the record and almost always external, usually climate, and the spread of Homo sapiens was the first time a single species precipitated a change of that magnitude.

The strongest counter-fact to growth as a goal is a gradient that runs the wrong way, and Part I quotes Harari at length for it. In Peru, Haiti, the Philippines and Ghana, developing countries suffering poverty and political instability, fewer than five people per 100,000 take their own lives each year, while in rich and peaceful Switzerland, France, Japan and New Zealand it is more than ten. South Korea is the within-country case: in 1985 it was relatively poor, tradition-bound and authoritarian with about nine suicides per 100,000, and having become a leading economic power with among the best educated citizens in the world and a stable liberal democracy, its rate is thirty-six per 100,000. Alongside this, depression is the leading cause of disability worldwide, more than one in ten Americans take antidepressants and one in four women in their forties and fifties, and prescriptions rose nearly 400 percent in ten years while the drugs lose effectiveness and are hard to stop. Part I's conclusion is the one that matters: economic growth is not a goal in and of itself.

The dominant disease class runs on four modifiable behaviours, which means the bottleneck is not biological. Part I cites a WHO report finding that the highest burden of non-communicable disease is imposed by conditions sharing the same four major risk factors: tobacco use, harmful use of alcohol, unhealthy diets and physical inactivity. Every one is a behaviour rather than a pathogen or a genetic endowment. That is why Part I treats the problem as a disconnect between modern lifestyles and evolutionary programming, and it is also why the problem is tractable in principle and intractable in practice: the four factors are each produced by industries that profit from them.

The new risk class has already overtaken the old ones, and this is the most consequential single fact in the health material. Part I quotes the Perlmutters: chronic disease accounts for 70 percent of American deaths, half of Americans suffer at least one chronic illness including diabetes, heart disease, cancer and Alzheimer's, and the World Health Organization now ranks chronic degenerative diseases collectively the number one cause of death on the planet, ahead of famine, infectious disease and wars. The transition Part I describes is not approaching, it has happened. Whatever we built to handle famine, plague and war is now addressing a minority of deaths.

Diet mortality is quantified three ways in Part I and the figures are large. Research from the Friedman School at Tufts indicates poor eating habits cause nearly one thousand deaths each day in the United States from diabetes, stroke or heart disease. A 2019 study in the Journal of the American Medical Association found increased consumption of processed food associated with a 14 percent increase in risk of all-cause mortality. And a Lancet study found that in 2017 one in five deaths globally were associated with poor diet. Part I also notes that poorer underlying health partly explains higher coronavirus death rates in America and Europe than in Africa.

Part I applies its own organised-complexity argument to medicine, and the structural point is strong. A core tenet of western allopathic medicine is that the body can be divided into siloes in which problems are treated individually while the rest is ignored, so a person's gut is treated as unrelated to their brain or heart. That ignores the interdependence between the body's systems, and the mismatch matters because evolution modifies organisms according to what works rather than what is simple. The incentive diagnosis is the sharper half: trained inside the modern medical system you learn that the best thing you can do is prescribe, not recommend a change of life, so to a hammer everything looks like a nail. This is Weaver's middle class of problems reappearing in clinical practice, which is the same objection Part I makes to prediction in economics.

The American plateau is two trends crossing, not one trend arriving, and this answers the objection that it is merely an opioid epidemic. From 1999 to 2009 the rising deaths from drugs, alcohol, suicide and organ disease were offset by large co-occurring reductions in mortality from ischemic heart disease, cancer, HIV infection, motor vehicle injuries and other leading causes. In 2010 the rising causes combined with diminishing further reductions in the falling ones to produce stagnation. By 2014 the rising causes more than offset the falling ones and life expectancy began to drop. So the deterioration had been running for a decade while the twentieth century's remaining wins concealed it, and what changed in 2010 was partly that those wins ran out.

The strongest evidence is a case where material life improved and health got worse. The post-war boom raised real incomes across the Western democracies and put refrigerators and televisions in ordinary homes for the first time, reducing absolute material deprivation, and over the same period the mortality disadvantage of blue collar workers actually increased in several countries. Absolute gain with relative loss produced a health decline, which no material account predicts. Part I draws the right conclusion: further growth can adversely affect a society's health if it happens alongside widening income inequality.

Part I states the healthcare chapter's bad basin in one sentence: a scenario in which the vast majority are absolutely better off because of economic growth, yet if a significant percentage of the value is captured by the top 1 percent, that growth produces a net decrease in life satisfaction and health. Growth is not merely insufficient in this account, it is capable of being self-defeating, because the distribution of the gains is itself an input to health. The claim is sharpened by Woolf: the price paid in health for not being on the top rung is getting more dire, so the stakes of position are rising rather than stable.

Three studies pin the causal direction, and the middle one is decisive. People whose houses were flooded in Bristol in 1969 had a 50 percent higher mortality rate than an unaffected control group over the following year. An Australian study of unemployment found the subjective experience of financial strain mattered more to health than actual income. And a study of a factory closure found that workers' health began to deteriorate when the layoffs were announced, before anyone actually lost a job. Anticipation preceded any material change, so in that case the causal arrow cannot run from material conditions to health. The pathways are specified rather than assumed: stress acting on the endocrine and immune systems, and stress raising the risk of smoking, heavy drinking, poor diet and inactivity.

The American reversal is quantified and the composition matters more than the total. Midlife all-cause mortality rose from 328.5 to 348.2 deaths per 100,000 between 2010 and 2017, and by 2014 midlife mortality was rising across all racial groups. Underneath: midlife mortality from drug overdoses rose 386.5 percent between 1999 and 2017, alcohol-related deaths rose through chronic liver disease and cirrhosis, and suicide among young and middle aged adults rose 38.3 percent, with the largest relative increase among children aged 5 to 14. The cause-specific increases began in the 1990s, two decades before the all-cause figure turned.

The size of the effect is what makes this chapter worth writing: between one half and three quarters of the difference in average life expectancy among developed countries is explained by differences in income distribution. The supporting pattern is that the longest-lived countries are not the richest but the ones with the flattest income distribution and the smallest share of the population in relative poverty, that among OECD countries the longest average life expectancies sit with the smallest income differences, and that among European Community members the fastest gains in life expectancy came with the fastest falls or slowest rises in relative poverty. If that magnitude holds, income distribution is the largest single lever on health in a rich country, larger than medicine.

The argument that settles it is not a statistic, it is the shape of the gradient. Health tracks socioeconomic status at every rung of the ladder: the rich are significantly healthier than the poor, and the super rich are healthier than the merely rich, and the slightly richer middle class are healthier than the slightly worse off middle class. Material deprivation cannot explain the top half of that curve, because the merely rich lack nothing material. A gradient that continues above the point where needs are met is evidence that position itself is doing the work, which is why the mechanism has to be psychosocial rather than nutritional.

Material living standards stop driving health at a point Part I can date, and the birth weight series is the cleanest marker. The proportion of babies born under 2,500 grams in England and Wales fell through the nineteenth and early twentieth centuries, then levelled at 6 to 7 percent in the 1950s and has stayed there, a residual core unaffected by better nutrition or sufficient calories. The conclusion Part I quotes: in the later stages of industrial development countries go through a health climacteric after which the health of the vast majority of the population is no longer substantially affected by the absolute material standard of living. Growth remains necessary to arrive and stops being sufficient on arrival.

Past a threshold the binding constraint on health changes from scarcity to comparison, and Wilkinson's mechanism is a shift of reference point. Income buys large life-expectancy gains while people lack necessities, the returns diminish as countries get richer, and there is a tipping point past which further growth has little effect. Wilkinson reads the epidemiological transition as marking the point at which the vast majority gained reliable access to the basic necessities of life. The mechanism Part I gives: while survival is the goal, survival is the yardstick; once necessities are secure people judge their lives against the lives of others, so general improvement is invisible because nothing has changed relatively. A rising tide lifts all ships, which is the problem rather than the consolation.

Capability converted famine from a fate into a failure, and the moral change followed the technical one. Part I: agricultural specialisation has converted famine from an unmanageable catastrophe to a human mistake. About 1.3 percent of the United States population, some 2.6 million people, produce enough to feed over 300 million and export the surplus, and the world produces substantially more food than is needed to feed everyone. So eliminating famine no longer requires more food, it requires better logistics and management, and if people still starve then someone has failed. Harari's marker for the shift is behavioural: instead of saying it was divine will, we convene boards of inquiry and resolve to do better. This is the general principle that power creates duty, shown once in history rather than argued abstractly.

The poor-law comparison is confounded, and Part I supplies the confound itself. Kelly and Grada: it was only because England was a relatively prosperous place that it was able to afford a generous system of public charity, and had living standards been as low as in France the English poor law would probably have been unaffordable. So France and Scotland were not countries that declined the institution, they were countries that could not buy it. The 1690s contrast therefore does not isolate institutions from productivity; agricultural intensification is upstream of both the relief spending and the resilience. The comparison still shows that a rich society can convert wealth into survival, which is not nothing, but it is weaker than a natural experiment.

The United States breaks the link between wealth and health, and that is the bad basin for the healthcare chapter. Part I's expectation is explicit: if economic growth creates and sustains better health outcomes, the most prosperous country should be among the healthiest and should keep improving. The 2019 JAMA finding is that United States life expectancy increased from 1959 to 2014, with the rate of increase greatest in 1969 to 1979, slowing thereafter, losing pace with other high-income countries, plateauing in 2011 and decreasing after 2014. A country can get richer and die younger, so economic growth alone is not indicative of better health outcomes.

The social gradient of the diseases of affluence reverses, and the reversal is evidence for the relative-deprivation account rather than decoration. Obesity was historically a status symbol, embodied proof of prosperity available only to the rich. After the transition these become primarily diseases of the poor, through a reversal in the social gradient in smoking and the consumption of refined foods such as white bread and sugar: as the products cheapen, income stops limiting consumption, consumption rises in the lower classes, and the status value of consuming them falls away. Coronary heart disease, stroke and obesity moved from businessmen to the poor without changing their biology.

The best-practice life expectancy frontier rose about three years every decade between 1840 and 2002, and Part I calls the consistency surprising. That linearity is the strongest single fact in the health material, because a straight line over 160 years through war, antibiotics, vaccines and sanitation suggests the gains are not driven by any one intervention. Part I then says the trend may be coming to an end, which is the claim that matters for the healthcare chapter and the one it does least to establish.

Famine killed through disease rather than through starvation, and the signature is a one-year lag. Kelly and Grada: before twentieth-century advances in public health, starvation-induced mortality was not an individual fate, because after bad harvests the poor migrated in search of work or charity, spreading disease and social disorder. Noble and commoner mortality correlate closely during famine, with noble deaths lagging commoner deaths by about a year, because bad harvests produced vagrancy, which multiplied disease vectors in a population already weakened by malnutrition. The poorest died immediately; everyone else died a year later of the epidemic.

The English poor laws are the cleanest natural experiment in the book, and they show a collective risk being bought down at enormous cost. By the end of the seventeenth century about 1 percent of national income went on poor relief, enough for complete subsistence for 5 percent of the population, rising to about 2 percent by the end of the eighteenth, against central government taxation of 3.5 percent of national income in the 1670s and 1680s. So relief consumed a large share of all state revenue. The payoff: in the 1690s bad weather caused crop failures across north-western Europe that killed an estimated 10 percent of the population in France and in Scotland, while England shared the poor harvests and escaped famine entirely. Same weather, different institutions, a tenfold difference in deaths.

Defeating famine and plague bought no extra lifespan, because the same growth that defeated them raised mortality by another route. Part I states it flatly: defeating famine and plague did not result in any increase in expected lifespan. Urbanisation produced crowding and poor sanitation; expanding trade circulated pathogens and achieved the microbial unification of the world; and higher density converted periodic epidemics into permanently endemic childhood diseases, concentrating mortality in the early years. This is the book's spine in its sharpest form: the engine that solves a risk class generates the next one, and the net gain was zero for roughly two centuries.

Cities were population sinks everywhere, not just in Europe. Until the late nineteenth century urban mortality was higher than rural mortality everywhere in the world, and in the eighteenth-century cities driving economic dynamism an excess of deaths over births was the norm, so cities could only hold their size through continuous migration of young adults from the countryside. The places generating the growth were consuming the people who made it.

The epidemiological transition is not a ratchet and can run backwards. Wahdan: health and disease patterns evolve in diverse ways through demographic, socioeconomic, technological, cultural, environmental and biological change, making it a continuous transformation process with some diseases disappearing and others appearing or reemerging. Reversals are common and follow changes in a society's underlying dynamics. This matters for the healthcare chapter: the transition is a direction of travel maintained by conditions, not a stage a country reaches and keeps.

Part I understates its own point by a factor of about ten. It says man-made risk outweighs natural risk by a factor of 100, so 99% of the risk to humanity comes from humanity. Ord states the comparison himself: he estimates anthropogenic risks to be more than 1,000 times more likely than natural risks, putting total natural risk at around 1 in 10,000 against total human-made risk of around 1 in 6. So the human-made share is over 99.9%. The corrected line is shorter and stronger than the original: Ord puts human-made risk at more than a thousand times natural risk.

An inflection point leaves no record to reason from, which is why outcome-bounding methods cannot guide us. Bounding total natural risk from the fossil and historical record works because natural hazards have run the same experiment for hundreds of millions of years. A transition has no such record by construction. So the method tells us what natural risk cannot exceed, which is why Ord can set natural threats aside, and tells us nothing about navigating the passage. Part I's demand for a forward-looking and deliberate approach is unanswered by Ord as well as by Part I.

We beat famine, plague and war without trying to. Part I: virtually nobody initially set out to combat these threats, because history taught that they were inexorable and a fundamental part of reality, and they were ameliorated instead as a byproduct of increasing specialisation in our economies. In the last hundred years, almost without anyone noticing, these threats were converted from unforeseeable catastrophes into preventable problems. The greatest achievement in human welfare was nobody's project.

The engine that solved the old problems is making the new ones, and the new ones hide. Part I: in the last 75 years specialisation created a class of risks that threaten our health and our species more than the age-old enemies, and because these are side effects of overspecialisation it is unclear that trial and error, our species' dominant method, can solve them. They build unseen for years before they explode symptomatically, and solutions are cheap, effective and less drastic early, but expensive, weak and invasive once the problem has inertia.

We are rolling the dice on our survival whether we acknowledge we are playing or not. Part I's formulation: we incur an incremental risk of extinction every year, that risk aggregates into a total risk load, the cumulative chance of extinction over time, and playing Russian roulette with the future of humanity is not a desirable option when the vast majority of the risk comes from our own actions. The gap between everyone wanting humanity to survive and what our actions actually produce is the signal that the economic system is not doing its job.

Ord's figures already assume we respond, and the gap between them is the part that belongs to us. His estimates incorporate the possibility that we get our act together; had he assumed business as usual they would have been substantially higher. His own image: we are heading toward a Russian roulette with two bullets in the gun, he thinks we will remove one before it is time to pull the trigger, and there might be time to remove the last one if we really try. So the headline should not be the risk he expects to remain, about 1 in 6, but 2 in 6, which is one in three: the difference between a lacklustre effort by humanity and a heroic one. Part I's upper bound of 1 in 2 should read 1 in 3. The 1 in 2 nearby is Ord's AI capability timeline, not a risk estimate.

The rich world's diseases come from solving the old problem. Part I: economic progress created an environment of plenty, so people in developed countries rarely die of starvation or material scarcity, and the new age of excess produced an epidemic of noncommunicable disease and mental illness that is harder and costlier to treat. Today more people die from diabetes than from malnutrition or starvation. The pattern is that a solved problem is replaced by a problem created by the solution.

Nobody chose agriculture, and after a while nobody could refuse it. Part I follows Diamond: domestication could not have been a conscious choice because the idea of a domesticated plant was alien to a hunter-gatherer, and the first steps were taken by people with no conception of the multigenerational process they were in. Then it locked: more efficient food production raised population density, and higher density demanded more production, an autocatalytic loop. Part I's conclusion is that increasing population densities had burned humanity's bridges and we could no longer go back. This is the book's cleanest case of a transition no actor decided and none could reverse.

Depending on the best option is what makes you fragile, and this is the same mechanism as the supply chains. Part I: foragers drew on a vast array of wild food sources, so they were resilient to disasters because they could switch from a species hit by drought or flood to one unaffected, while farming societies relied on a few domesticated plants and some on a single staple, which made whole regions vulnerable to a flood, a drought, locusts or mismanagement. Specialising on the most productive option removes the substitutes that absorb a shock. Agriculture changed who a risk belongs to, and that is why states exist. Part I: instead of individual risks like wild animals, personal violence, exposure, hunger and thirst, most people in agricultural societies die from collective risks, and widespread famines, plagues and the depredations of war are not the fate of individuals alone and cannot be solved by individuals acting alone. They needed coordinated and often centralised responses, and the inability of single people to reduce them contributed to the growth and spread of large political bodies with effective governance. The dominant risk class selects the institutions that follow it.
