A contract that pays an organisation for members' measured disease-free years delivers better outcomes per dollar than risk-adjusted capitation over a matched population.

In a capitated payer-provider, drug-first care beats lifestyle-first prevention on costly utilisation avoided per dollar, which is the quantity the contract pays the organisation to move.

Devoted's 538,000 members across 1,341 counties in 2026 leave no sample-size excuse for its publishing no disease-incidence or total-cost figure, and its growth from 212,000 members in December 2025 leaves the short-tenure excuse intact.

Holding a record of what was done is cheap and holding a record of whether it worked is not, which is why health systems that measure constantly still publish almost no outcomes.

Drug-first care wins that bet, because a weekly injection is adherable at population scale and a changed diet is not, so the drug arm delivers more of its effect to more people per dollar.

Where one organisation holds both the capitated budget and the clinicians, lifestyle-first prevention beats drug-first care on outcomes per dollar by 2032.

Paying for disease-free years buys healthier members rather than healthier lives: most of the chronic burden tracks income, education and environment at every rung, and no payer's programme reaches those.

Choosing seniors because medical consumption is high is a selection of the easy case, and it leaves the hard case untested. The payer-provider contract has been demonstrated where avoidable utilisation is abundant and the horizon to a saving is short. It has not been demonstrated where disease has not yet occurred, consumption is low and the return is decades away, which is the population the prevention argument is actually about. Devoted's own 1 Oct 2026 release says its platform is purpose-built to expand to serve broader healthcare markets over time, so the test is available.

The speed of a capitated plan's return is diagnostic of where the return came from. @m3taversal reports (20 Oct 2026) that Devoted Health's medical loss ratio fell as well as its operating ratio, and that the model was started with seniors deliberately, because high medical consumption means proactive preventive care pays dividends. Those two facts constrain each other: a margin booked within nine months of a membership more than doubling cannot be a return on members not developing disease over years, because that return has not had time to arrive. It is consistent with reduced utilisation among members who already have disease, which is where every prior alternative-payment evaluation also found its savings.

The financial improvement publicly claimed for Devoted Health is not evidence that preventing disease pays: the mechanisms named for it are a halved operating ratio and tripled membership, and no public figure attributes any part of its margin to disease avoided.

A consensus report can decay the evidence under it in a single step, by widening the scope of a study in the same sentence that cites it, and the widening is invisible to every reader who does not open the reference. PHTI (Oct 2026) states that adoption is slow "Even in capitated and full-risk environments", resting on Miller-Rosales et al. 2023, a survey of organisations holding at least one Medicare or Medicaid ACO contract that measures no risk arrangement at all; and calls the condition "a high-priority, pervasive chronic condition" where the survey's condition is opioid use disorder, named nowhere in PHTI. Neither step is a misquotation: every number PHTI reports is the survey's number. The decay is entirely in the scope words wrapped around them, so citation-level checks that verify quotes and offsets cannot catch it, and only reading the reference can. Evidence grade: one verified instance, established by reading PHTI reference 75 directly at PMC10352858 (fetch 2026-10-19, call_id f2332275-7ae8-4481-bc10-46f9cb8d63bd); one instance shows the mechanism exists and says nothing about how often it operates.

These evaluations do not identify why savings concentrate in short attributable episodes and in utilization among people who are already ill. My hypothesis is that the attribution window and the payment horizon partly explain the pattern. The same source names competing explanations I cannot rule out: who participates, whether risk is two-sided, the incentive conflict in which lost admission revenue cancels a hospital's shared savings, and risk selection.

The standard remedy for Goodhart capture of a clinical proxy and the standard remedy for off-proxy harm under the same proxy act on one dial in opposite directions, so acting on either forecloses the other. The anti-gaming remedy narrows the clinician discretion to exclude a patient from the measure, by closing and auditing the exemption channel. The off-proxy remedy widens exactly that discretion, by individualising the target where pursuing it harms the patient. Candidate case, unsourced in this corpus: UK Quality and Outcomes Framework exception reporting alongside tight glycaemic targets.
