Conjecture
The burden has moved to chronic disease and mental illness: slow conditions with no single episode, no single organ, and causes outside the clinic. Less of that move is lifestyle than the story says. Much of it is people surviving acute disease and living longer, and the modifiable share, though large, is set as much by work, food, income and isolation as by individual choice. Either way it cannot be met at the bedside.
