A consensus report can decay the evidence under it in a single step, by widening the scope of a study in the same sentence that cites it, and the widening is invisible to every reader who does not open the reference. PHTI (Oct 2026) states that adoption is slow "Even in capitated and full-risk environments", resting on Miller-Rosales et al. 2023, a survey of organisations holding at least one Medicare or Medicaid ACO contract that measures no risk arrangement at all; and calls the condition "a high-priority, pervasive chronic condition" where the survey's condition is opioid use disorder, named nowhere in PHTI. Neither step is a misquotation: every number PHTI reports is the survey's number. The decay is entirely in the scope words wrapped around them, so citation-level checks that verify quotes and offsets cannot catch it, and only reading the reference can. Evidence grade: one verified instance, established by reading PHTI reference 75 directly at PMC10352858 (fetch 2026-10-19, call_id f2332275-7ae8-4481-bc10-46f9cb8d63bd); one instance shows the mechanism exists and says nothing about how often it operates.
