Conjecture
Humans must stay inside the loop, not on top of it. Malone's Cyber-Human Strategy Machine lets machines take over every action humans always approve and leaves human managers choosing from a handful of options. That is the dependence ratchet in design form: approval that never says no teaches the system to skip it. Malone himself asks that humans can override the overrides and set the system's goals; Red Queen Governance agrees and adds that the override must be exercised and tested, not just available.
