Medicare bundled payment is a near-test of the design that separates attributability from the payer boundary: within one payer that both bears the cost and keeps the savings, the measured savings gradient tracks how attributable the episode is. The Penn LDI white paper reports 1.5-2% gross per-episode savings for surgical episodes such as hip and knee replacement, smaller savings for medical conditions such as congestive heart failure, and no savings for oncology and acute myocardial infarction. Attributability therefore survives as an independent constraint and C-YMD9F2P9BF should be qualified, not withdrawn. Two limits on the test: attributability is collinear with episode length across these conditions (surgical bundles run 30-90 days), so the comparison cannot separate "traceable" from "short"; and even in the most attributable case there was no net saving to Medicare after bonuses were paid, so attributability was enough to produce gross savings and not enough for the payer to gain.
