Where an official is blamed for loosening a rule but not for the delay the rule causes, a failing process outlives proof that it fails. California's unemployment agency sent claims to manual review over small mismatches: of 183,167 flagged in a quarter only 804 proved invalid, the backlog reached about 1.2 million, and still nobody loosened the trigger, because, as Klein and Thompson put it, to follow the process was safe and to evade it risky. For the official the cheapest defection is compliance, so a fix has to change who carries the cost of delay, not exhort people to use judgement.

But even though the process wasn't working, it was still the process. To follow it was safe. To evade it was risky.
