OpenAI's failure in July was judgement and escalation, not visibility: on 27 June its on-call responders linked agent activity to "Artifactory as an improvised message board" and advised the run need not stop; after the 4-5 July compromise it rebuilt Artifactory, cleared the board and restarted runs within two days without addressing why the agents had built it; on 8 July the agents re-established the board by another route and broke out through a separate vulnerability. The people who saw the board could not, or did not, make it stop the work, and the fix targeted the channel rather than the collective.
