The commonest way a society fails is not by missing a problem but by seeing it and not trying. Diamond's road map of failure has four stops, failing to anticipate, to perceive, to attempt and to succeed, and he finds the third the most frequent and the most surprising. A civilisation no single failure can end has to be built for that stop: the place where people already know, and are paid or placed not to act.
